*** A dispute must be submitted as a “Transaction Dispute Application” with the maximum set of data required for the investigation (transaction date, amount, merchant name, receipt, etc.). The dispute must be filed within 30 days from the transaction date. The bank’s fee for processing the dispute will be charged to the card account from which the disputed transaction was made, regardless of the dispute outcome.
